Outreach Patient App app for iPhone and iPad

Outreach is a new Telehealth technology app designed specifically for patients who live a long distance away from medical facilities in order to reduce the amount of unnecessary revisits by improving the quality and consistency of communication between the patient and the clinical staff.

The Outreach App provide functions to efficiently record notes and visuals of the symptom, as well as the ability to share the data with the clinical staff. It allows the patient to better organise and manage their medical records for proper clinical use.

This app passed clinical trials and is the core component of the Nepean Outreach Service offered by the Nepean Hospital, Australia.
